Patient is a 63 year old admitted to Greenery Center for Rehab and Nursing after a 2 week hospital stay for acute respiratory failure. Prior to her hospital stay, she was at home with help with shopping and transportation. Upon admission patient required moderate assistance for self-care, including bathing, dressing, and grooming, minimal assistance for all bed mobility and transfers, and severe decrease in activity tolerance. She was on 3L oxygen with frequent episodes of desaturation with activity. She also experienced poor insight to safety noted with mild confusion. After just a 3 wee length of stay patient was able to discharge to a personal apartment, independent with ability to safely manage oxygen needs. She was walking more than 200 feet without any assistive device, demonstrating good carryover and understanding of respiratory education provided at facility to allow maximum quality of life.
