Patient is a 43 year old female admitted with significant decline in mobility due to complications including +C-diff, alcohol abuse causing diffuse brain atrophy and hypobalemia. Guillain Barre had been ruled out. Prior to hospitalization patient was completely independent in daily activities, held job, and performed all activities of home care. Patient admitted to Greenery Center for Rehab and Nursing 10/19/2016 requiring moderate assistance for all self-care including bathing, dressing, and grooming. Maximal assistance required for all mobility (supine to sit, sit to stand, and all functional transfers) and was unable to wall:?. After a 6 wee!:? length of stay of hard work and great participation, patient was able to safely return home and to work independently. Patient left facility performing all aspects of self-care independently without adaptive equipment and walking 300+ feet using a rollator walker.
