Patient is a 69 year old suffering from multiple fractures from a fall. Upon admission to Greenery Center for Rehab and Nursing, she required maximum assistance for all aspects of self-care including bathing, dressing, and grooming. Assistance was also required for bed mobility, standing and all transfers. Initially patient was able to walk 10 feet with moderate assist, however she was very fearful of falling during therapy sessions. After a few sessions with the therapist, she gained great trust and was able to make remarkable gains in a short period of time. After a four wee length of stay she was able to safely discharge home with her sister walking 125 feet using a straight cane and supervision, transferring independently, and performing all aspects of self-care with supervision.
