Restrictions to Protect Residents in Skilled Nursing Care Facilities Temporarily Remain in Place in Counties in the Green Phase of the Governor’s Process to Reopen Pennsylvania

Washington County has moved into the Yellow Phase due to a 5.3% COVID Positivity Rate. Due to the rise into the Yellow Phase, the Greenery will be temporarily suspending visitation for at least 3 weeks. The facility will monitor the Positivity percentage rate and based on those numbers will dictate when visitation will resume.\

The facility has had 37 staff vaccinated for COVID and all of the current Physicians and Nurse Practitioner that round at the facility have been vaccinated.
The facility has 66 residents vaccinated for COVID.

The facility tests employees weekly at this time.

Lobby visits with residents have begun. Visits are M-F 10A-2P and W-Th 5-7P. Visits can be scheduled by calling the Administrator or Director of Nursing.


April 6th, 2021

The facility has had 62 residents vaccinated for COVID.
The facility has had 34 staff vaccinated for COVID and all of the current physicians and Nurse Practitioners that round at the facility has been vaccinated.

The facility tests employees weekly at this time.

The facility is now scheduling in person visitation! Please contact the receptionist at 513-351-7007 in order to schedule a visit with your loved one. We also currently have the ability for zoom calls and window visits. These can be scheduled via the Activity Department.

Last week the Greenery Center completed COVID-19 testing of all employees and residents.  All tests came back negative.  No staff or residents are showing any signs of COVID.  We want to thank all residents, families, and staff for their understanding and patience during this trying time, while we try to maintain a COVID free building.

June 2, 2020
As counties enter the Green Phase of Governor Wolf’s Process to Reopen Pennsylvania, congregate care restrictions remain in place. Skilled nursing care facilities (SNFs) licensed by the Department of Health (Department) are congregate care facilities. Per the Centers for Medicare and Medicaid Services’ Nursing Home Reopening Recommendation for State and Local Officals, the Department is following the recommendation of choosing a longer waiting period after the counties in which the SNFs are located enter the Green Phase before relaxing restrictions in those SNFs. Due to the nature of the vulnerable population living in SNFs, the restrictions put in place by the Department’s Interim Guidance for Skilled Nursing Care Facilities During COVID-19 shall remain in effect for at least 28 more days after the beginning of the Green Phase in those counties.
To develop a well-informed, manageable plan that ensures a measured approach for the safe return to activities, visitation, and other events for residents of SNFs, the Department is conferring with stakeholders that include the state’s ombudsman, representatives from SNFs, nursing home associations, other government agencies, and academia. Upon completion of that process, the Department will issue guidance that amends the restrictions currently in place.
